Preparing for Your Physician Visit

Thank you for choosing the physicians of Pioneer Medical Group as your partner in healthcare! A good physician-patient relationship is essential for your continuing good health. You can help develop your relationship by preparing in advance for your scheduled office visits.

As an added convenience to our new patients, we now offer downloadable patient registration forms that you can print and fill out at home before your first appointment. Use the links below to locate the Patient Information Form and the Health History Form. By using these links, please print the forms and completely fill out each form prior to your first appointment.

Since you are new to Pioneer Medical Group, you can request to have your previous medical records sent to your physician's office prior to your appointment. Your PMG physician will want to know your past medical history including information regarding past illnesses, conditions and treatments. Listed below is a link for a Medical Release form. Please print out the form, completely fill it out and then forward to your prior physician's office. This will help to ensure that your medical records are received prior to your first appointment with your PMG physician. Also, if your child is a new patient to PMG, please bring their most up-to-date immunization record.

To Prepare for Your Appointment

1. Please arrive 15 minutes prior to your appointment. We value your time and want to keep you on schedule too.
2. Please bring your insurance card, driver's license or other picture ID, along with the required co-payment for your visit.
3. Please complete a Patient Information Form
4. Please complete a Health History Form
5. Bring a list of your current prescriptions, including any over-the-counter medications that you are taking.
6. Bring a list of allergies or any adverse reactions to mediations that may have occurred in the past.
7. If your appointment is for a physical and is scheduled in the morning, please do not eat or drink anything other than water for at least 12 hours prior to your appointment. This will help to ensure that all necessary tests can be done on the same day and prevent a return trip to our office.

If you are unable to download and print items 3 and 4 please arrive 30 minutes before your appointment to allow time to complete the forms.
